**Role purpose**

The Case Manager will provide a high quality and culturally appropriate case management support for participants in the Ngarra Jarranounith Place program.

**Program description and Role Key Responsibilities**

Ngarra Jarranounith Place translates to Men’s Healing Place in Woi Wurung language.

The program is 16-week residential service designed to support men who use or are at risk of using family violence to make positive changes in their lives. The program is 16-week residential service designed to support men who use or are at risk of using family violence to heal and make positive changes in their lives. The program offers intensive one-on-one support and group activities that focus on supporting and addressing:

- Strong Spirit and Strong Culture
- Taking Responsibility
- Healthy Relationships

The program’s unique and holistic approach focuses on planning for the participant’s transition back into their community and where possible linking men into the Men’s Healing & Behaviour Change Programs located across Victoria.

**The key responsibilities of the Community Worker are**:

- Coordinate and contribute to the planning of behavioural change and healing activities that align with culture, healthy relationships and responsibility
- Conduct a culturally appropriate assessment and intake for participants who have been accepted into the Ngarra Jarranounith Place Program
- Providing high quality and culturally appropriate case management support for participants in the Ngarra Jarranounith Place Program
- Engagement of participants in intensive case work designed to address identified risk factors as well as individual goals

- Working with participants to develop exit plans (addressing housing, education/employment, health and wellbeing) and step-down support
- Contributing to the review of intake and assessment processes for participants referred to the Program, and decisions about men’s suitability and readiness to participate in the Program

- Develop, manage, review developing and update managing individual care plans for men and supporting and problem solving with them men to achieve their goals, access services they need, attend appointments, meet Court Orders, etc.

- Working closely with the Partner and Family Support Worker, to keep the family safe and support partner contact and access to children, if appropriate.
- Support men during the workshop camp or other healing activities, including their attendance at the Men’s Behaviour Change Groups, participation in volunteering, engaging in community events and building trust within the Aboriginal community.

**Service delivery/ Practice Framework**
- Maintain program awareness and presence by developing collaborative working relationships with diverse stakeholders in both statutory and non-statutory settings.
- Participate in core capability training, action learning and practice reflection opportunities to expand practice skills.
- To engage and build positive and constructive relationships with program
- Work effectively as part of a team, contributing to group outputs and reflective practice.
- Actively participate in regular supervision with the line manager, staff meetings and professional development opportunities.

- Ensure all written and verbal communication is professional and appropriate for the purposes of carrying out the responsibilities of your role and in accordance with the professional standards and values of the organisation.
