Clinical Care Team Leader | Permanent Full Time Position

We are currently recruiting for a Clinical Care Team Leader to join our team on a permanent full-time basis at Regis North Fremantle WA.

Reporting directly to our Clinical Care Manager, you will be responsible for providing and coordinating resident care, and supervising care staff to ensure this beautiful home has an effective, efficient and functioning team.

**Duties & Responsibilities**
- Lead and participate in the delivery of a high standard of person-centered resident care.
- Provide a high standard of ongoing care management and perform nursing interventions.
- Coordinate the comprehensive completion of individual holistic assessments and care plans for new and existing residents.
- Ensure that the coordination of all clinical documentation is completed accurately and in a timely manner.
- Assist Clinical Care Manager with monitoring of the medication management by reviewing medication charts and medication storage.

**About you**

**As a collaborative and strategic member of the Clinical Leadership Team, ideally with experience within the health or aged care sector, we require you to have**:

- Current AHPRA Registration as a Registered Nurse.
- Knowledge and relevant experience in clinical services in a complex nursing or aged care setting.
- Understanding or ability to learn the attributes of the Commonwealth Funding System in relation to ACFI.
- Ability to provide direction and expert advice in the treatment of acute care episodes.
- Demonstrated understanding of the relevant Laws, Acts, Regulations, Charters and Codes of Practice.
