We are working with a respected in home care services organisation who are looking for a Clinical Care Manager to join their North Sydney office in a full-time permanent opportunity.

In the Clinical Care Manager, you’ll use your clinical, customer service, and care management skills to make an impact on your clients and their needs

**The Role**
- Coordinate in home clinical care needs
- Managing care plans through clinical judgment, creativity, and sensitivity to develop care plans that are aligned to the client’s assessed needs
- Develop, monitor, reassess, and documentation with tailored care plans in consultation with clients, their families, and other healthcare professionals
- Work in collaboration with clients and their families manage to facilitate and develop service offerings.
- Manage and assist with incidents and complaints to ensure resolution

**About you**
- Registered or Enrolled Nurse
- Passion and desire to support families while displaying
- Empathic and caring nature with the ability to build strong relationships
- A full Australian drivers license and access to your own car
