**The opportunity**:
In the role you will be working with a diverse mix of people who really care about making a difference for people and the NSW community and you will be provided with ongoing professional development and peer support. You will have a mind for thinking holistically and set the bar high through the quality of work that you do. You will also ensure person centred, timely, high quality and cost-effective services are coordinated and provided to meet participant’s needs and scheme objectives.

We’re looking for someone with knowledge, practical experience and skills, to support participants and their families with a disability or allied health background to manage a caseload of participants with complex injury related needs as they transition back into their community following serious injury, including brain and spinal injuries.

It’s an exciting time at icare where no two days are the same; as a Rehabilitation Coordinator in the Northern team you will:

- Use a person-centred approach to assist in identifying and reviewing the needs of participants across all phases of recovery and community participation.
- Facilitate supports to enhance each participants’ capacity to achieve their desired goals, as identified in their My Plan.
- Work collaboratively with internal specialist services, external case managers, health professionals and other key stakeholders to coordinate the most effective options to meet the treatment, care and support needs of the participant.
- Manage service approvals and payments in accordance with agreed needs, assessments and plans.
- Adhere to the KPI’s that monitor and measure key areas of the role
- Keep abreast of and understand the legislative framework and Guidelines and how these contribute and impact the decision-making framework.
- Work collaboratively with all teams across Care, in the design and implementation of internal quality assurance, management of disputes and complaints and continuous improvement systems to drive quality and consistency in decision making

**Your story so far**:

- An allied health background (or a relevant degree in a health-related discipline or rehabilitation advisor), you are equipped with transferable skills to coordinate rehabilitation services to people with disabilities similar to those supported by our schemes.
- Experience with working with people with traumatic brain injury and/or spinal cord injury would be required
- Practical experience in managing a caseload of participants, determining treatment planning needs and coordinating the relevant services and support that the scheme funds. You’ll use your critical reasoning skills to ensure a high degree of integrity in the decision-making process, have excellent communication skills to build rapport and truly connect with people to build trust and familiarity with participants, their families and their rehabilitation team.
- Excellent communication skills give you the ability to build rapport, communicate complex information, create, and nurture effective working relationships with all stakeholders. Previous injury management experience working with people who are severely injured is highly advantageous.
- A full driver’s license and ability to travel regularly within the Northern region of NSW

**What we offer**:
We know our strength comes from the diversity of our people and so we want people with different experiences and backgrounds to apply. We recognise the importance of balance and flexibility and understand that it’s not a one size fits all approach.
