**Monash Health Social Work Program**

The **Social Work Service** at **Monash Health** is dedicated to assist patients with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. This includes the delivery of a broad range of social work services for public and private patients and their families across the lifespan in the inpatient and outpatient setting. Social Workers function as vital members of a multidisciplinary team and have the pivotal role where social complexity exists to ensure a high quality, evidence-based service to all patients and their families at **Monash Health**. Our **Social Work Department** is growing with new **Grade 2 Social Worker **position** **now available at the heart hospital.

**Social Worker Grade 2 - Critical Care and Cardiac Services**

Excellent opportunity for an experienced **Grade 2 Social Worker **to be part of a dynamic and inspiring social work and broader allied health team working in a collaborative model of care across **Monash Health** and **Jessie McPherson Private Hospital**. **About You**

As an accomplished **Social Worker**, you take pride in working collaboratively with a range of stakeholders including Grade 3, Clinical Grade 4 and Social Work Managers, to provide current evidence-based practice for patients accessing care through the inpatient and community-based setting.

To ensure your success in this role, you will have the following demonstrated skills, experience and knowledge:

- Bachelor Social Work or Master Social Work (Qualifying)
- Skilled clinician with consolidated psychosocial assessment and planning skills and able to use evidence to inform practice
- Understanding and working knowledge of the range of the service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, and the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model is essential
- Eligible for AASW membership including continuing professional education
- Leadership skills to supervise and mentor clinical staff including Grade 1 Social workers, Allied Health Assistants and students, and contribute to service development
- Delivery of effective, high-quality care to patients across the heart hospital and Monash Health
- Provision of safe best practice and quality care with unwavering commitment to patient centred care
- Flexible and enthusiastic approach in line with the AASW practice standards
- Excellent communication skills and ability to work closely with relevant clinical teams across Monash Health to promote optimal service delivery and patient care in a ch
