**About the role**

We are currently recruiting for an experienced Team Leader to look after the health and wellbeing of our clients through the provision of high quality service in a supported independent living and community environment. This position has come up because of a restructure as we are continuously growing. As a Team Leader, you will be responsible for working with participants to develop and implement an Individual Support Plan, and through facilitation and supervision of direct support workers to achieve the agreed support arrangements and participant’s goals.

**Duties and responsibilities may include, but is not limited to**
- Using a person-centred approach, build and maintain relationship with clients and stakeholders.
- Assisting senior management with prioritising participants and developing relevant internal policies and procedures to undertake agreed action and achieve outcomes respecting their choice.
- Ensure Support Workers are communicated with effectively from the recruitment process and beyond.
- Development and implementation of individual plans with participant/nominee. Monitoring of plan outcomes and meeting audit requirements.
- Managing SIL Houses and their rosters.
- Ensure excellent in client service delivery, including managing assessments of client care, goal planning and the success transitioning new participants into CMG houses.
- Work on a rotating on call roster to manage urgent out of hours issues.
- Conduct house & client audits to ensure with compliance with the NDIS legislative requirements.
- Lead in the implementation of CMG policies and procedures.
- Maintain client records and database, and ensure staff are reporting accurate progress notes.
- Managing client escalations and complaints to a high level of resolution
- Manage all risk management issues and ensure compliance with relevant policies.
- Experience in the Team leadership role and managing SIL delivery
- Current CPR & First Aid certification.
- National Police Check & NDIS Worker Screening Check.
- NDIS Workers Orientation Module.
- Valid Australian Drivers Licence & Own vehicle.
- Excellent communication and interpersonal skills.
- Certificate in Leadership & Management
- Certificate in Disability Services/Community Services or relevant.

**Salary**: $38.00 - $45.00 per hour
