**Level 2 Support Coordinator (Part-Time with Potential for Full-Time)**

**About Us**
Wish Support Group is a dedicated NDIS service provider based in the North of Melbourne, Victoria. We have an exciting new opportunity for an **Experienced Level 2 Support Coordinator** to join our team in a part-time capacity, with the potential to transition into a full-time role.

**About the Role**
As a Level 2 Support Coordinator, your primary responsibility is to empower individuals with disabilities to engage with community and mainstream services while achieving their personal goals as outlined in their NDIS plans. Our aim is to support participants and their families/carers in building capacity to manage their plans and supports independently.

**Key Responsibilities**
- Develop and maintain proactive relationships with NDIS-registered and non-registered providers.
- Monitor participant plan outcomes and manage expenditure effectively.
- Take a holistic approach in coordinating, implementing, and maximizing participants' NDIS plan goals.
- Maintain clear, accurate, and timely record-keeping for compliance purposes.
- Work collaboratively with our accounts team.

**Essential Criteria**
- Must have **at least 2 years of experience** in NDIS Support Coordination or a Community Case Management role.
- Passionate and enthusiastic about the NDIS and supporting participants.
- Strong liaison and networking skills to maintain knowledge and build referral contacts.
- Ability to support participants in understanding and implementing their NDIS plan.
- Excellent written and verbal communication skills.
- Strong ability to work collaboratively and independently.

**Additional Qualities**
- Patience and empathy when working with participants.
- Strong negotiation and networking abilities.
- High-level interpersonal skills with a proactive and adaptable approach.

**Requirements**:

- 100 Points of identification.
- A valid Driver's License and comprehensively insured motor vehicle.
- Valid NDIS Worker Screening Check.
- Working With Children's Check.
- Full Australian working rights.

**Job Details**
- **Job Type**: Part-time (with potential for full-time).
- **Pay Rate**: $35.00 - $45.00 per hour.
- **Work Location**: Hybrid remote in Broadmeadows, VIC 3047.
