We are seeking an experienced, highly skilled Team Leader who is excited by new challenges and passionate about leading our new Case Management team to support clients to achieve individual goals.
About our Case Manager Coordinator role and employee rewards:

- Permanent part-time position - hours to be negotiated
- Annual salary range - $88,761 to $92,773, commensurate with qualifications and experience, plus 10.5% superannuation
- Excellent salary packaging options that allow you to spend $18,550 of your pre-tax salary on everyday expenses (eg. rent, mortgage, groceries, school fees, etc) and entertainment (eg. eating out), significantly increasing your take home pay. What you will be responsible for as the Case Manager Coordinator:

- Case management coordination for the Southern New South Wales Local Hospital District’s (SNSWLHD) Out of Hospital Care program
- Administering and monitoring OHC program workflow
- Ensuring OHC Team is meeting KPIs
- Mentoring and supporting a team of OHC Case Managers across the SNSWLHD
- Facilitate successful relationships with SNSWLHD staff and CS#1’s service brokers
- Facilitate successful referrals to other service providers

To be successful in the Case Manager Coordinator role, you will:

- Demonstrate experience in leading a team to deliver program and client outcomes
- Demonstrate experience, or ability to, successfully leave a remote team
- Demonstrate ability to work with clients with diverse cultural, emotional and social needs with health goals ranging from rehabilitation to disability support to end of life
- Have strong interpersonal, engagement and relationship building skills
- Be available to travel across NSW, as required
- Hold, or be willing to obtain, record of vaccination (in accordance with the NSW Ministry of Health Occupational Assessment, Screening and Vaccination Against Specified Infection Diseases Policy Directive, Category A role requirements)
- NSW Working with Children Check
- National Police Check
- ACT Working with Vulnerable People Card
- Current Driver Licence and own vehicle
- Current First Aid Certificate
- Up to date COVID vaccination status
