About the role
The purpose of this role is to provide a Specialist Palliative Care Consultation Service across inpatient and outpatient services at Ipswich Hospital. Specialist palliative care services in West Moreton are delivered through an integrated model of care with all aspects of the service working collaboratively toward patient centred care across the continuum as the patient moves seamlessly through different settings of care. The consultation service provides specialist palliative care advice for inpatients, patients in the emergency department and outpatient clinics.
About you
- Demonstrated interpersonal skills which allow effective communication in a multi-disciplinary environment.
- Demonstrated knowledge of safety and quality assurance initiatives, as well as the ability to conduct audits and data analysis of key outcome measures.
- Experience in relevant areas of outpatient care and community care.
- A sound knowledge of contemporary Human Resource issues including time management, Workplace Health & Safety, Equal Employment Opportunity, Anti-discrimination and Code of Conduct.
**Mandatory Requirements**:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
