**Description**:
Promote the values of CORE to provide clients with independence, participation, care, self-fulfilment and dignity. Ensure awareness and support of the philosophies of the person-centred approach, healthy ageing and independent living and upholding the rights of participants.

The Case Manager / Support Coordinator will coordinate and implement supports from a range of sources to strengthen participants’ abilities to self-direct their supports and services. The Case Manager / Support Coordinator will do this by establishing a positive collaborative relationship with the participant, their family and/or carer in identifying and connecting with appropriate providers of support. With a strong understanding of Aged Services and the NDIS you will ensure the implementation of participants’ plans and achievement of their goals to support each to overcome barriers, build skills and participate more fully and independently in the community.

Together with the Senior Program Manager, the role champions the collaborative development of lean and seamless internal processes and cohesive systems that support holistic service delivery and ensure that participants consistently receive exceptional service and high-quality service outcomes.

Responsible for ensuring the Aged & Disability Program plan, deliver care and services in accordance with relevant funded program guidelines, Disability Act 2006, NDIS framework, Aged Care Quality Standards and CORE Policy and Procedures.

**Responsibilities Include**:

- Promote empowerment, choice and control and community inclusion of people with disabilities, older people and their families, consistent with their individual plan
- Shares expert, quality and responsive case management advice, guidance and develops skills and knowledge and builds capacity within the Aged & Disability Program
- Monitor and evaluate service delivery, including but not limited to regular internal audits of client files and services.
- Manage allocated workload effectively through effective planning, organisational and time management skills
- Responsible for the performance of the Aged & Disability Program against key performance indicators, ensuring that all team members comply with documentation, and statistical data reporting requirements
- Contribute to the development, monitoring and review of policies and procedures pertaining to consumer safety and quality services.
- Monitor individual participant budgets to enable choice and empowerment
- Provide Support Workers with timely advice, support and information based on best practice, risk management, NDIS Commission Guidelines, Aged Care Quality Standards, the NDIS Code of Conduct and CORE Policy and Procedures
- Participate in accreditation standards and practices activities in conjunction with the Program Support Coordinator.

Support older people and NDIS participants to build capacity to coordinate their individual plans, negotiate appropriate support and services, and connect with community support and mainstream services.
- Review and maintain all Service Agreements and ensure that Service Agreements dates adhere to Service Booking dates on PRODA.
- Actively marketing and promoting CORE Aged & Disability Program to new prospective Participants and service providers.
- Actively participate in the on-call process
- Provide support to across the Home Care package program and Home Support Program as required
- Any other tasks as required by your manager

**Selection Criteria**:

- Relevant tertiary qualifications or equivalent
- Demonstrated experience in a community care setting that includes advanced and current commercial knowledge and skills.
- Awareness of the Aged Services and the NDIS including issues facing older people and people with Disability
- Superior case management skills and previous experience in a similar role
- Proven knowledge of the Aged and Disability framework including the National Disability Insurance Scheme Act 2013 and Disability Act 2006, Aged Care Quality Standards and other relevant legislation.

Strong communication, customer service, negotiation and influencing skills

Excellent ability to connect and build rapport with multiple stakeholders
