**Job Title: Disability Support Worker**
**Location**: Fairfield VIC

**Job Type**: Subcontract
**Pay**: $33 - $45 per hour
**Expected Hours: 2**0 hrs to 30 hrs

**About the Role**:
We are seeking compassionate, dependable, and dedicated **Support Workers** to become valued members of our team. In this role, you will have the rewarding opportunity to make a real difference in the lives of individuals living with disabilities and/or mental health challenges.

As a Support Worker, you will be responsible for delivering **person-centered care**, tailored to meet the unique needs, preferences, and goals of each individual. This includes offering both emotional and practical assistance to help them build their confidence, develop life skills, and achieve greater independence in their daily lives.

You will support individuals in a variety of settings—whether in their homes, supported living environments, or out in the community. Your duties may include helping with daily routines, encouraging participation in social and recreational activities, assisting with personal care, and promoting overall well-being.

**Key Responsibilities**:

- Provide personal care and support with daily living activities
- Support clients with community engagement and skill-building activities
- Promote health, well-being, and independence
- Follow person-centered support plans and communicate effectively with team members
- Ensure compliance with Work Health & Safety and NDIS standards
- Maintain accurate and timely records of support provided

**Qualifications & Requirements**:

- Certificate III in Individual Support (Disability) or equivalent experience
- Current First Aid and CPR Certificate (or willingness to obtain)
- Current Australian Driver’s Licence
- Valid ABN
- NDIS Worker Screening Check
- National Police Check and Working With Children Check (or willingness to obtain)
- Immunisation Certificate (COVID-19 and others as required)
- Physically fit and emotionally resilient to meet the demands of the role
- Must have access to a reliable vehicle

**Desirable Skills and Experience**:

- 1 to 3 year experience in providing personal care or in-home/community support
- Experience supporting people with disabilities or mental health conditions
- Understanding of person-centered and active support approaches
- Strong problem-solving skills and ability to work both independently and in a team
- Excellent communication and interpersonal skills, including active listening
- Comfortable with basic technology (MS Word, Outlook, data entry, etc.)

**Shift Availability**:

- Morning shifts
- Afternoon/evening shifts
- Overnight shifts
