**Scope**:
As a Support Worker at Cessible Healthcare, you will play a crucial role in delivering personalised care and resources to our participants. Your aim will be to assist them in achieving their goals and maintaining a comfortable and fulfilling lifestyle.
**Roles and Responsibilities**:
- Possess comprehensive knowledge of NDIS support items and adhere to the NDIS Guidelines.
- Provide direct assessment and support to clients while following Fellowship policies and procedures.
- Foster positive working relationships with community agencies, clinical services, and relevant providers for effective communication and care coordination.
- Support clients individually or in group settings to make informed decisions, overcome obstacles, and work towards their goals.
- Advocate for clients to access necessary community services.
- Monitor and report on participant referrals, progress against NDIS Goal Plans, and collect relevant data for reporting purposes.
- Complete mandatory administration tasks, including accurate data entry into our client management system.
- Engage in ongoing professional development as required.
- Perform any other reasonable duties as directed from time to time.

Security Check Requirements:
- Worker’s Screening Check.
- Police Check.
- Working With Children Check (if applicable).
Qualifications and Memberships:
- A minimum of 1-year prior work experience is preferred.
- We will provide you with the necessary training to perform your role effectively.
