**About the role**
The Clinical Care Partner plays a pivotal role in ensuring the delivery of high-quality, culturally sensitive, and person-centred care under the Support at Home (SAH) program. Working collaboratively with non-clinical Care Partners, this role provides clinical expertise and comprehensive assessment to ensure participants receive safe, effective, and tailored support that maintains their independence and wellbeing at home. The Clinical Care Partner is also responsible for managing a personal caseload of participants with restorative care needs and/or end-of-life (EoL) pathway packages, including all related service coordination and participant budget management.
**Key Responsibilities**
Your main responsibilities will include, but not be limited to:
- Manage an allocated caseload of participants, including those on Restorative Pathway and/or End-of-Life (EoL) pathway packages, ensuring tailored and holistic care.
- Conduct comprehensive clinical assessments and complete documentation in line with relevant standards, legislation, and ANHF policies.
- Receive referrals from non-clinical Care Partners to review participants’ clinical needs, undertake clinical assessments, interventions, and provide recommendations or suggestions for care planning. Escalate complex or high-risk cases to the Clinical Care Manager for additional support and oversight.
- Develop detailed, individualised clinical care plans that consider participants’ clinical needs, goals, preferences, and existing supports.
- Regularly review and adjust care plans to reflect changing health conditions and participant preferences. Coordinate services with multi-disciplinary teams, including GPs, allied health, palliative care, and external providers, to ensure seamless care delivery. Develop strategies in consultation with participants, families, and supporters to address clinical and personal needs within funding frameworks.
- Maintain oversight of participant budgets, ensuring appropriate and effective use of funding in line with program guidelines.
- Promote ANHF’s values and ensure culturally appropriate care practices are embedded in all aspects of service delivery.
**About the Successful Applicant**
- Essential:_
- **Registered Nurse with current AHPRA registration**:
- Minimum 5 years’ experience in aged care
- Strong clinical assessment, care planning skills
- Understanding of culturally sensitive practices within CALD communities.
- Understanding of restorative and palliative/end-of-life care approaches.
- Proven ability to work collaboratively with multi-disciplinary teams.
- Strong problem-solving and critical-thinking skills.
- Ability to work independently and manage competing priorities.
- Empathy, patience, and cultural awareness.
- Commitment to continuous improvement and innovation in care delivery.
- Knowledge of Support at Home (SAH) program guidelines and aged care reforms.
- **Fluent in English and Cantonese/Mandarin.**:
- Excellent communication, education, and advocacy skills.
- Proficiency in electronic care management systems and Microsoft Office Suite.
- Desirable:_
- Strong knowledge of palliative and EoL care, with proven case management experience
- Good understanding of HCP and SAH, including funding and compliance requirements.
- Experience in supporting and guiding colleagues in managing clinical issues.
- Good understanding of the different approaches within RACF and SaH contexts
- Good understanding of the new Aged Care Act and Strengthened Aged Care Quality Standards
