**Role Details**

The Practitioner, Specialist Family Support Pathways is a role within the Exceptional Needs Unit and is accountable to the Manager, Specialist Support Pathways for:

- assessing children, young people, and families for eligibility for the Voluntary out of Home Care (VoHC) pathway
- case managing and the implementation of VoHC with children and young people
- working collaboratively with clients, families, networks, and carers to improve support provided to people exhibiting behaviours of concern in a way that is respectful of peoples' human rights, puts the client at the centre of planning, and aims to improve quality of life for clients with exceptional needs
- providing high level consultancy, advice and reporting on individuals with exceptional needs and other populations with complex and unmet needs requiring support to live in the community
- facilitating and supporting the development and implementation of appropriate community-based support responses for people with exceptional needs, especially in the VoHC cohort.

Key outcomes and accountabilities:
1. Undertake assessments, coordinate cases and provide a systemic service for children and young people in the VoHC pathway.
2. Build, maintain and collaborate with stakeholders within government, non-government and private sectors.
3. Support and assist children, young people and their families within the VoHC pathway to navigate services and advocate where required, to meet their needs.
4. Provide professional support and leadership to other discipline employees within the team and professional supervision and support of student placements as appropriate.
5. Manage client information systems and maintain high quality case notes and reports.
6. Maintain awareness of relevant trends and issues and best practice within the State, nationally and internationally.
Any other responsibilities in line with the classification level of the role as assigned by Line Manager and/or the Department. The responsibilities as specified above may be altered in accordance with the changing requirements of the role.

- Must hold a current Australian issued driver’s licence (equivalent to minimum class C).

**Essential Qualifications**
Incumbent must hold
- an appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers; or
- an appropriate degree or equivalent in Bachelor of Disability and Developmental Education, which enables eligibility for full membership of Developmental Educators Australia Incorporated; or
- an appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with Occupational Therapy Australia; or
- a degree level qualification in Psychology and registration with the Australian Health Practitioner Regulation Agency.

**About the Business**

Community and Family Services supports vulnerable people and families to participate in social and economic life and build stronger communities. This division comprises three key areas:
**Safer Family Services (SFS) **provides a wide range of intensive family support services to children, young people and families experiencing a high risk of statutory intervention from the child protection or youth justice systems. Services also include family support, community development and parenting and youth programs.

The **Office for Women **works across government and the community towards achieving gender equity and positive change for women in South Australia by developing policy and advising on issues affecting women to improve their safety, wellbeing and economic security. The office also oversees the Women’s Information Service to support women over the phone, online and in person, as well as regional safety hubs across the state.

**Communities and Justice **works towards building community safety and wellbeing by providing support and services to children, young people and families in the youth justice system, building the capacity of communities through community development projects and working in partnership with the community services sector to respond to people with complex and exceptional needs.

**Salary Range**: $85,665 - $99,241 (AHP2)
