Role will be to help in different departments to provide education and community access to people with intellectual, physical, mental illness, social and emotional disabilities.

**To be considered for this role you must have**:

- AQF Associate Degree, Advanced Diploma or Diploma
- Minimum of 3 years of experience in similar role.
- You must be Australian Citizen/ Permanent Resident or have proper working rights in Australia.

**Duties and responsibilities**:

- Interviewing clients to assess their level of difficulties and building rapport with them
- Making assessment of clients' needs and creating the training and support programs accordingly
- Preparing the report on the health progress of the client/patients
- Maintaining the notes and reports for all other routine tasks
- Monitoring the health program prepared for client/patients
- Referring clients to other support groups that can provide additional help to clients
- Liaising with other health care professionals and community groups
- Helping people/clients especially young people who have emotional, social, and financial issues
- Building supportive relationship with people with mental illness, their carers and families
- Using person-cantered approach work with people with mental illness to maximise their independence and confidence to stay in community
- Helping in preparation and submission for financial funding to support clients
- Monitoring delinquents and people on bail as needed
- Helping the Centre with the assessment of community needs in areas such as health, housing, employment, training etc.
- Take responsibility to work in accordance with workplace health and safety guidelines and follow the Policies and Procedures of the organisation

**Job Types**: Full-time, Permanent

**Salary**: $55,000.00 - $60,000.00 per year
