**Our Purpose**

United for Care offers independent living for members of the community to live in individual apartment accommodation with 24 hour support in our Vertical Villages. The team at United for Care are passionate and dedicated to innovation in the disability sector with the purpose of improving the quality of life for the vulnerable and people living with disability.

**The Position**

The **Home and Community Support Manager** for the **Drop-In Support Program** is responsible for overseeing and coordinating all aspects of the program, ensuring high-quality day care and support for participants. This role involves managing a team of support staff, coordinating Participant Risk assessments, developing care plans, transitioning participants into our services, and ensuring the ongoing day to day smooth operations of the Service.

The Home and Community Supports Manager will work closely with Participants, their families, and other stakeholders to deliver personalised supports to meet the individual needs of each participant.

The role will report through to the Head of Service Delivery and will work in close collaboration to ensure the Drop-In Support Program is developed and implemented effectively.

We are seeking an experienced motivated individual who has:

- A Bachelor's degree in Social Work, Nursing, Healthcare Management, Community Services or a related field (Master's degree preferred).
- Proven experience in project and program management, preferably in Drop in Supports; home care disability or healthcare services.
- Strong leadership and team management skills.
- Excellent communication, collaboration and interpersonal abilities.
- Understanding of both physical and psychosocial disabilities.
- Proficiency in budget management and resource allocation.
- Knowledge of relevant laws and regulations related to home care services (eg: NDIS Standards)
