**Are you an experienced clinician ready to take on a leadership role in a dynamic healthcare environment?**

Join our dynamic healthcare team in the role of **Access and Community Brokerage Manager**

We are seeking a highly skilled professional to provide clinical expertise for individual patients and leadership across your specified area of practice within our innovative Care of the Older Person and Community Transition (CO-ACT) Unit.

In this role, you will:

- Deliver and oversee expert clinical care to individual patients.
- Provide leadership and guidance to the Access and Community Brokerage Multidisciplinary team.
- Coordinate and lead projects or programs that enhance patient, client, and service outcomes.
- Exercise significant autonomy in clinical decision-making.
- Cultivate strong relationships with both internal and external stakeholders of CALHN to ensure a seamless patient journey into and out of the COACT program.
- Accept accountability for your team, including the outcomes for patients or client groups, and ensure consistency with policy.
