Great Ocean Road Health is seeking a Diabetes Educator in a part time capacity for 8 hours per week. The Diabetes Educator provides high quality diabetes education, clinical management, and information and support to clients and staff. The role includes primary health care focused assessment, planning, early detection, referral, monitoring and maintenance and direct care to individuals, families and groups in relation to diabetes. The Diabetes Educator will also take an active role in the coordination and provision of health promotion activities.
- ESSENTIAL CRITERIA, APPLICATIONS AND CLOSING DATE

**Essential Education**:

- Bachelor’s degree or higher in a health related field and completed a post graduate degree in diabetes education
- Credentialled or working towards accreditation by the Australian Diabetes Educators Association
- Current NDIS workers check

**Desirable skills**:

- Self motivated and able to work without supervision
- Ability to act in an advisory role to management
- Proficiency in group therapy programs
