**Are you looking for a new and exciting challenge?** We are looking for a **_Team Leader_**_ _based within **Melbourne**Outer East**. The role of _Team Leader_ involves both leading and mentoring a team, as well as directly supporting people with disability when required. You will have an understanding of the dynamic nature of support work and a dedication to minimising the risks associated with the service, you will provide effective individualised and tailored supports to NDIS participants to enable them to achieve their goals. You will do this whilst considering input from all stakeholders associated with each participant. These supports will promote community inclusion, independence, decision making, choice and control. You will be responsible for delivering support and services to a caseload of participants under the National Disability Insurance Scheme (NDIS). You will take responsibility for the delivery of a high quality of service, referrals, rostering, professional development and supervision of a team of Support Workers and positive participant outcomes. Through NDIS in Victoria, WISE provides supports for participants with current funding in their NDIS plans.
