**About Us**:

Our multidisciplinary team is committed to improving the health and well-being of our patients through a holistic approach that addresses physical, mental, and emotional needs.

**Position Overview**:
We are initially looking for an individual to be between our Royal Park and Salisbury locations as well as spending 1 day per month with each of our Barossa and Victor Harbor teams and clients. With the plan to create multiple separate rolls in these locations as the team further expands.

**Key Responsibilities**:

- Conduct comprehensive assessments of veteran patients to identify their physical, psychological, and social needs.
- Develop, implement, and monitor individualized care plans in collaboration with on-site healthcare providers, patients, and their families.
- Coordinate and facilitate access to healthcare services, community resources, and support networks.
- Advocate for veteran patients to ensure they receive appropriate and timely care.
- Provide counselling and support to veterans and their families, addressing issues such as PTSD, substance abuse, and other mental health challenges.
- Assist patients with navigating the VA system and accessing veteran-specific benefits and services.
- Maintain accurate and up-to-date patient records and documentation.
- Participate in multidisciplinary team meetings to discuss patient care plans and progress.
- Conduct outreach and education activities to increase awareness of available resources and support for veterans.
- Stay informed about best practices and advancements in veteran care and social work.

**Qualifications**:

- Bachelor or Masters degree in Social work
- Full Australian working rights
- Mental health social workers preferred but not essential

Expected hours: 20 - 38 per week
