**Join Our Team as an Experienced Intake Worker**
Monash Health Aged Care Services is thrilled to announce an exciting opportunity for a highly skilled and experienced **Intake Worker** to join our **Community Support Options** team on a full-time basis.
**About the Role**
As a key part of Monash Health Aged Care Services, our **Community Support Options** team delivers essential services to carers and care recipients across the municipalities of Casey Cardinia, Greater Dandenong, Kingston, and Bayside. This role is integral to our **Home Care Package (HCP)** program and **Commonwealth Home Support Program (CHSP)** as we transition to the **Support at Home Program** under aged care reforms.
In this role, you will:
- Facilitate access to vital aged care services, enabling clients to live independently in the community.
- Provide up-to-date service information and assist clients with navigating online tools to access sustainable, appropriate services.
- Act as a central point of contact for clients, ensuring seamless referral management and support.
**What We Are Looking For**
We are seeking a dynamic professional with extensive experience in aged care and a passion for making a difference in clients’ lives. To succeed in this role, you will have:
**Essential Experience**:
- Proven track record in delivering direct support to **Home Care Package (HCP)** clients as a current or previous HCP Care Manager.
- Hands-on experience supporting **CHSP** clients as a current or previous CHSP Coordinator.
- Expertise in referral management as a central point of access worker in a medium to large healthcare or aged care setting.
**Qualifications & Skills**:
- Relevant tertiary qualifications in health or social sciences.
- In-depth understanding of aged care reforms and client-directed care models.
- Strong computer literacy and proficiency in online service tools.
- Excellent communication, organisational, and interpersonal skills.
- Ability to work autonomously while thriving in a collaborative team environment.
