**About the role**

We are looking for an experienced Disability support worker to join our team and provide support to our clients in community Participation support access and skill building in **MULGRAVE, and surrounding Suburbs.**

As a support worker in our team, you will be responsible for supporting individuals with daily living skills, assisting with social and recreational activities, implementing and monitoring practices that will promote a healthier lifestyle, empowering participants to help them achieve their goals. The role will involve working with participants with both low and complex needs. Some may need assistance with cooking and meal preparation and other day to day activities. Experience and professionalism are highly recommended.

**Additional Duties for this role include**:

- Personal Care.
- Social and Community Participation.
- Assisting with shopping.
- Transportation.
- Cooking and meal preparation.

**Key Responsibilities Include**:

- Build rapport and collaborate effectively with staff/management, participants, their family, and friends to achieve a positive outcome and ensure a smooth service delivery flow throughout the participants' plan.
- Increase participants skills with your active support and promote independence by enhancing their skills and personal development.
- Assist with liaising with participants families and friends to explore new goals, ambitions, promote and empower participants to achieve their goals, and provide a medium of solid communication.
- Provide regular and frequent quality and professional support in line with participants plans, needs, and wants.
- Implement strategies and programs to develop/enhance participant skills, identify areas of increased support and opportunities for services to help achieve their goals.
- Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle.

**What you will need**:

- Minimum Certificate III or IV in Aged Care, Home & Community Care, Individual Support or Disability Services
- Current CPR with First Aid Certificate
- NDIS Worker Screening Clearance
- Current Australian Driver’s License
- Reliable, registered, and insured vehicle (comprehensive insurance is preferred)

**Salary**: $35.00 - $45.00 per hour

Schedule:

- Flexible hours
- Rotating roster
- Weekend availability
