**Position Overview**:
**Key Responsibilities**:

- Provide general household assistance, emotional support, and companionship to clients.
- Assist clients with personal care tasks, including bathing, dressing, grooming, and toileting.
- Support mobility needs, including transfers and the safe use of assistive equipment.
- Assist with meal preparation, serving, and feeding if required.
- Perform light housekeeping duties, including cleaning, laundry, and maintaining a safe environment.
- Facilitate community participation by accompanying clients to medical appointments, social activities, and recreational outings.
- Administer medication as per individual care plans and in accordance with relevant guidelines.
- Observe, document, and report client progress and any changes in condition to relevant healthcare professionals.
- Implement individualized care plans in collaboration with healthcare providers, families, and support networks.
- Promote independence by assisting clients in developing and maintaining life skills.
- Ensure compliance with workplace health and safety standards and NDIS guidelines.
- Provide emotional and psychosocial support to clients and their families, fostering a positive and supportive environment.

**Skills and Experience Required**:

- Certificate III or IV in Disability, Individual Support, or an equivalent qualification.
- Minimum of **a year** experience in a disability support, aged care, or community support role.
- Strong understanding of **NDIS** and person-centered care principles.
- Current **First Aid and CPR certification**.
- National Police Check and Working with Children Check (or willingness to obtain).
- Valid **Australian Driver’s License** (preferred).
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work both independently and as part of a team.
- Compassionate, patient, and dedicated to supporting individuals with disabilities.
