What your day will look like....
Your day will involve supporting discharge planning by liaising with Aged Care providers, helping patients and families complete documentation and coordinating practical arrangements like transport or emergency financial support. You'll collaborate with Social Workers and Clinicians to ensure care needs are understood and matched appropriately, contributing to smooth, respectful transitions. Every action you take is focused on building trust, ensuring clarity and creating moments of dignity in what can otherwise be a daunting process.
