Our **Moving Moments Program** is a government funded program that brings seniors who are living independently and children of all ages together to share in fun, play-based activities supported by professional educators and artists.

**About the role**:
As a **Support Worker - Home Care Packages/Moving Moments Program**, you will be responsible for:

- Delivering services for clients at home and in the community as outlined in the Personal Independence Plan including supporting and guiding clients to complete identified tasks, as well as completing agreed tasks on behalf of clients.
- Supporting clients to participate in activities and excursions as part of the Moving Moments Program
- Contributing to assessments of the client’s goals, interests and needs.
- Maintaining accurate comprehensive case notes.
- Undertaking administrative duties associated with the role.

**We have casual and part time opportunities available - Monday - Sunday 25 hours per week.**

**About you**:
Critical to your success in this role will be the proven ability to demonstrate:

- A high level of effective interpersonal and communication skills
- Your ability to maintain confidentiality and impartiality
- Respect and responsiveness to client needs
- Well maintained, clean & reliable vehicle with full comprehensive insurance
- A willingness to work with non-traditional approaches to service delivery for older people

**To be successful in this role it is **essential** that you**:

- Hold a Certificate III (or higher) in Supported Living (Aged Care)
- Maintain a valid, unrestricted Qld Driver’s licence
- Have a well maintained, registered and insured motor vehicle
- Have intermediate level computer skills
- Are fully vaccinated for CoViD19 (including boosters)
- Maintain a NDIS Worker Screening check

An interest in Allied Health Assistance would be highly desirable.
