**ABOUT THE ROLE**:
**Summary of the role**:

- Provide high quality adult Social Work services across acute, sub-acute, palliative and outpatients settings to a variety of clients including those with complex social needs and chronic health conditions. Work as a valued member of the large Allied Health Team to promote and develop social work services within the Hilltops Council area.
- Permanent full time position at Mercy Care Centre public hospital, Young NSW- Provide evidenced based Social Work assessment and interventions to a varied adult client base
- Sole practitioner allowance is applicable to this position
- Subsidised hospital accommodation available to the successful applicant and relocation asssitance may apply.
- Deliver comprehensive services to our adult clients incuding general counselling, advocacy, advice and education, linking with care providers and discharge planning. Supervision and professional development supported.

**ABOUT YOU:
**- Bachelor of Social Work (or equivalent) and eligibility for membership with the Australian Association of Social Workers
- Current NSW Drivers Licence
- Demonstrated time management and organisational skills
- Enthusiastic, flexible, positive team member who is focussed on best practice and quality care
