**About the role**

The organisation are looking for an passionate & dynamic Social Worker to join their highly regarded and diverse aged care program team to develop and deliver comphrensive assessments to understand the needs of your clients.

You'll have the opportunity to manage a diverse caseload, catering to a wide range of therapeutic demands, facilitating seamless discharges, plan future care plans, counselling to enhance quality of life for all clients.

**Key Selection Criteria**
- Relevant tertiary qualification in Social Work (or equal equivalent) with the eligibility for membership with the Australian Association of Social Workers (AASW)
- National police check, WWCC and a drivers’ license essential
- Well developed assessment & therapeutic skills
