Trieste Foundation is a registered NDIS service provider supporting people with a disability and mental health condition in the Central Coast, Newcastle, Lake Macquarie, Maitland and up Hunter region.

**Job description**
**Position Objective**:
This position is responsible for providing support to individuals with a disability under the National Disability Insurance Scheme (NDIS). This position supports individuals with their disability support needs (including individuals with high physical support needs, Intellectual disability, psychosocial supports and behaviours of concern).
Support staff would need to have flexible availability for a range of shifts across a 24/7 roster, this may include sleep over or active shifts and weekends.
**Primary role responsibilities**:
- Demonstrate a commitment to rostered hours by negotiation with the Manager
- Commitment to supporting individuals to live independently by supporting with in home support, personal care and community access.
- Engage positively with participants and always promote their best interests, safety and wellbeing
- Implementation of support plans and behaviour support plans
- Support participants to make own choices
- Engage in training as required
- Always maintain professional boundaries.
- Manage health and safety: Support participants to look after their health. Take action and manage any health and safety risks to them or yourself.
**Operational Tasks and Responsibilities**:
- **Health and Wellbeing** Encourage and support participants ongoing safety and developmental wellbeing, including promoting self-esteem, appropriate health, diet, exercise, hygiene, sexual and personal development. Support participants to implement health and allied health support plans
- **Assistance with daily living tasks -**This includessupport with healthy meal preparation, domestic chores e.g. clothes washing, cleaning dishes, household cleaning (mop and vacuum floors), garden maintenance and day-to-day household tasks as directed, assistance with shopping, packing and unpacking groceries.
- **Community Access** - Support community access by encouraging community participation in community activities and promoting positive interactions and inclusion with others.
- **Assistance with personal care needs** i.e. showering, shaving, toileting
- Maintain positive and respectful relationships with the families and significant others within the participants’ support networks.
- Implement participants’ i.e. Individual Support Plans, Behaviour Support Plans. Health Care Plans, Mealtime management plan, medication management plan.
**Manual Handling Tasks** - physical support with transferring from bed to chair, assistance with sit stand position, repositioning in bed, using sling hoists and transfer equipment, managing movement of wheelchairs, support to move in and out of vehicle, adjusting electric bed, use of slide sheets and boards, high adjustable tables
- Participate in ongoing training and professional development to increase skills to care for participants.
**Medication Assistance -**Support participants to implement their medication plans, document medication administration
- Work collaboratively as part of a team
- **Record and Incident Reporting** -Keep accurate records, files and attend to reporting requirements in a consistent and clear manner in line with Trieste Foundation Policy and Procedures.
- Work always in accord with NSW legislation, NDIS Standards and code of conduct as well as Trieste Foundation WHS and other policies and procedures.
- Perform any other duties as may be required from time to time that are consistent with the Support Worker and Agency responsibilities.

- Competent writing skills and verbal communication skills.
- Understands professional boundaries and abides by the NDIS Code of Conduct.
