Deliver flexible and holistic case management response to clients

The Salvation Army Youth Services offer an integrated suite of programs engaging with young people across Australia. Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

THE ROLE

We are seeking a values-driven and proactive Youth Services North/West Metro Case Manager to coordinate all secondary consults for clients of the North West Metro service.

Responsibilities include:

- Deliver flexible and holistic case management response to clients with professional and consistent case management to those experiencing homelessness or at risk of experiencing homelessness
- Ensure that young people within the North West region receive an appropriate service response that upholds The Salvation Army’s Model of Care, Journey Towards Independence
- Coordinate Referrals and intake of all clients across all
- Manage and engage in active assessments of young people
- Complete comprehensive intake and risk assessment for all young people, incorporating adequate assessment of the needs of accompanying children where relevant.
- Assist young people with a range of activities in preparing for independent living
- Provide case management support to a caseload of young people who are experiencing high and complex needs

HOURS AND SALARY

This is a permanent full time position, 38 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

ABOUT YOU
- Tertiary qualification in social sciences, welfare or related field with a degree qualification preferred. Relevant experience in a social service environment
- Understands and identifies indicators of physical, emotional, psychological or sexual abuse in infants, children and adolescents
- Knowledge of youth homelessness, crisis intervention and the ability to collaborate with other program areas
- Demonstrated skills in engaging & working with young people with complex support needs including AOD, mental health, out of home care and adolescent violence
- A current VIC Drivers Licence
- Valid VIC Working with Children Check
- Ability to provide proof of Eligibility to Work in Australia
