As a Support Coordinator you will work in conjunction with Service Partners Team Manager to establish and maintain positive and productive relationships with internal and external stakeholders in the region to ensure the effective delivery of an array of services to See Differently clients.

We’re looking for a real team player with excellent communication skills to provide non-clinical care and practical support to our clients who have a NDIS plan in order to strengthen their abilities to coordinate and implement supports and participate more fully in the community.

You will enable clients to feel empowered by assisting them to identify and set goals and support them to establish positive relationships with services providers as well as addressing issues of social isolation and supporting their independence within their community.

This role is essential in our organisation.

**Your duties will include**:

- Provide Services including advocacy, planning, referrals, and yearly review reports.
- In partnership with the clients, design, co-ordinate and implement an agreed plan to meet goals.
- Act as the primary point of contact for consumers, carers, and service providers.
- Coordinate services as defined in the participant’s NDIA plan, using a strength
- based and capacity building approach. This will include informal supports, mainstream services (housing, health), specialist services, and NDIS funded supports.
- Develop and monitor service agreements in consultation with clients and service providers.
- Conduct a needs assessment with the participant, based on their NDIS goals and the level of service defined in the NDIA plan.
- Provide support to consumers and carers as appropriate and develop strategies to resolve consumer issues.
- Monitor and maintain outcome focussed and up-to-date client records in accordance with professional standards, RSB policy and procedures and relevant legislation.
- Establish and develop relationships with key community and government services.
- Work collaboratively with specialist staff to provide holistic services for RSB clients.
- Develop an accessible resource file directing clients to various support agencies locally, that can connect people who are blind, or vision impaired with available community resources.
- Participate in appropriate community networking meetings as required.

Occasional travel may be required.

**Your skills and expertise**

Essential
- Current driver’s licence.
- Current satisfactory DHS screening and National Police Clearance or willingness to obtain.
- Certificate IV or above in Disability Services, Social Services, or related field
- Prior experience in case management/support coordination and/or providing professional and flexible assistance to individuals with disabilities.
- Extensive experience in the development of person-centred planning and the ability to demonstrate a clear understanding of support facilitation.
- Ability to work independently, take initiative and respond effectively to a wide range of competing work priorities.
- Ability to work collaboratively and communicate effectively with multiple stakeholders, including health professionals, support facilitators, family members, carers, and other internal service providers.
