Melville Cares currently seek a Home Care Package Coordinator with excellent people skills, high emotional intelligence and strong aged care industry experience. As an organisation in growth mode, you will coordinate, manage and optimise client packages, manage a team of Community Workers and work to ensure the best care is provided to clients, in line with the values and purpose of Melville Cares. The successful applicant will have sound knowledge and understanding of home care services and guidelines and the aged care standards.

**Included in this role is the ability to**:

- Identify client’s needs and goals and develop client directed, goal-based support plans that incorporate a reablement and wellness approach;
- In partnership with the client, establish, monitor and manage the client’s Home Care Package budget;
- Liaise with referral agencies, service providers, consumers, family/advocates, carers to ensure best possible outcomes for clients;
- In partnership with the client, monitor and review their progress against agreed goals and outcomes and undertake / arrange assessments as identified, during the client’s journey;
- Manage a team (up to 15) of Community Workers; and
- Complete documentation in line with, and comply with, guidelines, contractual obligations and legislation.

**Criteria**:

- Demonstrated experience in leading a team to achieve desired outcomes.
- Demonstrated knowledge of service provision, coordination, allocation and compliance with contractual obligations to ensure quality service provision.
- Well-developed communication skills both verbal and written, and interpersonal skills to develop effective relationships and resolve client and staff issues.
- Demonstrated decision making and problem-solving skills.
- Knowledge and understanding of the Aged Care standards.
- Knowledge and understanding of the reablement approach to care.
- Knowledge and understanding of the quality improvement process.
- Knowledge of client eligibility criteria and assessment processes.
- Demonstrated computer / Microsoft Office skills incorporating data base managed services.
- Sound knowledge of WHS Act and procedures.
- Demonstrated time management and organisational skills to effectively plan and prioritise workload.
- Full COVID-19 Vaccination Certificate
- Current Police clearance, no less than 3 months old.
- Current “C” Class licence with clear driving record and comprehensive insurance.
- Certificate III in Aged Care or above
- Willingness to actively participate in training opportunities to both maintain required competencies and develop additional skills to enhance service delivery.

**Salary**: $70,000.00 - $80,000.00 per year
