**Description**:
About the role
As a key member of the ATSICHS Brisbane team, the NDIS Support Worker is expected to personally contribute to the shaping and achievement of ATSICHS vision and goals within their own scope.
The NDIS Support Worker will work with individuals (known as ‘participants’) in partnership to empower and build their agency to make effective change and overcome their identified disability and wellbeing needs. The role is heavily outreach based, engaging participants in their homes, and translating initial engagement into a strong ongoing partnership. Once engaged, the role will support participants in daily living activities and social/community participation. All supports provided are aimed at promoting participant choice and control. Accordingly, a key focus of the role is building the capability of participants, not completing goals/supports transactionally to the participant.
The support we provide is diverse and the NDIS Support Worker will be helping participants in their own homes, within group care homes and assisting them with daily tasks and activities that may include:
- Daily activities, such as getting out for a coffee or catch-up with friends, participating in a hobby or sporting activity, going on day trips or holidays.
- Support with medical appointments and any other support that may contribute to helping them achieve their goals while staying healthy, happy and safe.
- In addition, when required this role will provide relief support as a transport driver for the Early Childhood Department.
Duties
- Meet individual case performance targets as per weekly, monthly and quarterly dashboard system.
- Provide high quality and intent-based practice in all interactions with clients, consistent with the ATSICHS values and vision.
- Provide high quality case management practices and record keeping that adhere to operational requirements.
- Demonstrate positive and warm engagement through an assertive outreach approach that engages clients into the service.

**Skills and Experiences**:

Essential
**Skills and Knowledge**:
- Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
- Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
- Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams.
- Understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.
- Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing.
- A passion for supporting people with a disability to achieve their NDIS goals and supporting them to participate in Community, Social and Civic activities.
- National Police Clearance.
Qualification and/or Experience
- Certificate III in Disability or other relevant qualification.
- Previous experience of working with people with a disability.
- Requirement to travel across the Logan and/or Brisbane region, as per program location.
- Current drivers’ license.
- Flexibility to work outside core 8:30am-5pm business hours in meeting individual client needs and provide responsive care.
- Working with Children’s Card (Blue Card) or be eligible to apply.
