**About the Carer Gateway Program**
Wellways Australia is the first point of contact for all Australian Government Carer Gateway funded services across NSW, aiming to help Carers access practical information and advice and connect with services in their local area.

Carer supports and services will be delivered using a network of local Carer Gateway regional service providers Carers and those who provide a caring role to someone in their life who has a disability, mental health issue, chronic health condition, terminal illness or are frail-aged.

**Your new role**
As part of the dynamic Carer Gateway - Mobile Carer Support Team you will be required to engage with both Carers and Community Referral Partners to increase engagement in the Carer Gateway Program.

Working with Carers you will assist then to identify their goals and plan for the delivery of appropriate practical and core service supports and facilitate the opportunity for them to share experiences and learn from each other in a peer group setting.

Working with Community Organisations you will use your extensive relationship building skills to build and maintain effective referral pathways for carers and to raise awareness of carers within the broader community.

The successful applicate will require the following attributes:

- Assessment and planning skills
- Group Facilitation experience
- Partnership and relationship building skills
- Experience in working with people who have complex needs is highly desirable

**Pre-employment compliance requirements**
- Certificate IV or above in a support discipline and/or Equivalent Community sector experience
- Lived experience as a young Carer or adult Carer
- Evidence of working rights within Australia (birth cert/passport/visa)
- Satisfactory National Police Records Check (within the last 12 months)
- Working with Children's Check / Blue Card
- NDIS Worker Screening Check
- NDIS Worker Orientation Module Certificate - free online course
- COVID 19 Vaccination Certificate with Booster or valid medical exemption
