**Key Responsibilities**:
- Provide one-on-one support to clients with various disabilities in their daily activities, promoting independence and skill development.
- Assist clients with personal care tasks, including bathing, grooming, dressing, and medication management, while respecting their dignity and privacy.
- Collaborate with clients, their families, and other professionals to create and implement individualized support plans.
- Engage clients in social and recreational activities that align with their interests and abilities.
- Monitor and document clients' progress, behaviors, and any changes in their health or emotional well-being.
- Maintain a safe and clean environment for clients, ensuring their safety at all times.
- Encourage and empower clients to participate in decision-making processes related to their lives.
