Are you an experience Care Worker, keen to progress your career to a Case Manager opportunity?

Our valued not for profit client has opportunities for a new graduate or experienced care worker seeking a case management role in Home and Community Care. This position will be site based in the eastern suburbs to support onboarding, coaching and development in the role. However the organisation also has flexible working arrangements and would consider Full time or a 0.8 arrangement.

Your role will involve the following:

- Completing Care plans and assessments for consumers
- Assisting consumers and families with referrals and accessing services that meet their needs
- Monitoring consumers care needs and implementing services
- Networking with Regional assessment teams, external providers and community support services
- Rostering support services, and intake of new consumers

We are seeking expressions of interest from self starters who present with well developed interpersonal skills and a customer focus. You will also demonstrate the following:

- A Certificate III or IV in Aged Care with relevant industry experience in home care or relevant Diploma or Tertiary degree.
- An understanding of consumer directed care and funding options in community aged care.
- Well developed time management and ability to prioritise work effectively
- A team focus and ability to foster strong relationships internally and external of the organisation.
- Sound problem solving skills and flexible approach
- Proficient in MS Office (Outlook, Access, Word and Excel)
