**Belmont Hospital**

**About the role**:
Our Social Workers deliver interventions to patients throughout the hospital, including in the Medical and Surgical Wards and the Emergency Department.

In this role, you will be available to work on Thursdays and Fridays with flexibility offered for the remaining day.

**Where you’ll be working**:

- Belmont Hospital is situated at the northern end of Lake Macquarie surrounded by picturesque views, coastal walking tracks, family friendly parks and lakeside picnic areas and award-winning restaurants and cafes. - You will become part of our small and supportive team where each member plays a crucial role, contributing their unique skills and insights to create a collaborative and nurturing work environment.
- Our team comprising of a Level 4 Team Leader, Two (2) x Clinical Specialist Level 3 staff and an early career Social Worker.

**Requirements**:

- Must hold as a minimum a bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.

**About you**:

- You will have the ability to develop a greater understanding of NDIS and RACF processes, support patients with complex care needs and be organised and flexible in your approach.
- You will be able to provide clear documentation on your interventions and patient care plans and communicate succinctly with the multi disciplinary team.
- You will be organised and able to provide handover to other staff for patients who require follow-up on the days you are not working.

**Additional information**:

- This role commences in December 2024.
- An eligibility list will be created for future temporary part time vacancies.
- Interviews are tentatively scheduled for the week commencing Monday 23rd September 2024.
