About the Role
The Mental Health Triage clinician role is pivotal to the Wembley Road community facility for the first point in contact for people seeking assistance with:
- Mental Health Triage and assessment for walk in consumers.
- Responsible for planning and coordination of care following assessment or discussion and with the medical team.
- Responsible for daily duty intake Monday - Friday 0830-1700pm (last assessment 1600pm).
- Referral pathways to internal and external teams following ad hoc review of referrals held daily.
- Assist the team with consumers that may be on the duty intake register for immediate tasks for follow up.
- Receive verbal handovers from other teams/services regarding Transfer of Care and liaise with Team Leader for action.
- Medication assistance for consumers that attend the clinic for Long Acting Injectables (LAI's).
- The Clinical Nurse plays a pivotal role in providing in person support for the attainment of consumer goals including the development of independent living skills.
