Looking for multiple Care Mangers with all levels of clinical experience to join one of Victoria's leading age care/ndis providers to deliver exceptional quality care within the Aged Care and Disability Sectors.

**What is on offer?**

**Your day to day duties will include**
- Delivering clinical and other care and services to clients as set out in their plan of care and services
- Adhering to all requirements and obligations of the Aged Care Act, the NDIS and associated Regulations and Standards in the delivery of care.
- Directly supporting and providing patient diagnosis, treatment, testing, and ongoing care.
- Supporting participants to remain living independently within the community and manage new intake of consumers/participants
- Maintain accurate and up to date case notes on Consumer’s/Participant's progress
- Conducting family consultations in relation to care plan/NDIS plan evaluations with consumer’s/participant’s family, representatives and medical decision-making representative.
- Observe consumer/participant physical, emotional and behavioural conditions, document and keep the Team Leader-Aged Care & NDIS informed of any changes

**As the successful Clinical Care Manager you will be**:

- Tertiary qualification in Health or a related field and relevant experience in care management evidenced
- A current Victoria driver’s license, National Police check, NDIS Worker Screening Check, Working with Children Check and First Aid with CPR
- Excellent verbal and written communication and interpersonal skills, including report writing
- Familiarity with community aged care/NDIS services and referral pathways
- Well-developed prioritization and organizational skills with the ability to meet deadlines
