**About the role**
The role of the Social Worker is to work within the Mission and Mission in Action of HammondCare ensuring a high standard of Social Work service is provided to patients of the Hospital, along with appropriate support and advice to their carers.
The Level 1/2 Social Worker is offered on a Permanent Full-time basis and is located at Braeside Hospital, NSW.
**The essence of this role**- Provision of effective and efficient social work service to clients, including appropriate support and advice to their families and carers- Participate as a member of the multi-disciplinary team in assessing patients’ level of functional and care needs, setting care goals and providing appropriate intervention.-
- Provide the provision of the highest standards of quality care.
**About you**- Commitment to align and engage with HammondCare’s Mission, Motivation and Mission in Action- A tertiary qualification in Social Work, providing eligibility and membership to the Australian Association of Social Workers. Qualifications obtained overseas must provide AASW Overseas Assessment number.- Practical knowledge and clinical skills in delivering Social Work services to Rehabilitation, Aged Care Psychiatry and/or Palliative Care setting.- Demonstrated interpersonal, consultation and reasoning skills necessary to ensure high clinical practice standards and contribute to strong team practice, using a variety of media - oral, written and computer.- Demonstrated skills and experience in interacting constructively and collaboratively with the internal multidisciplinary team and external service providers.- Experience in quality improvement initiatives aimed at enhancing service delivery, including planning, development, implementation, evaluation and professional education and training.- An unencumbered Driver’s License and willingness to travel between HammondCare’s sites and services.

- Demonstrated experience in an acute care setting as a social work clinician or student placement.
- Demonstrated effective verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences.
- Demonstrated commitment to professional development, evidence based practice and continuous improvement.
- Demonstrated ability to work as a member of a multidisciplinary team.
- Ability to learn and utilise a variety of computer programs and work in an increasingly electronic workplace.
- Eligibility to drive in NSW and ability and willingness to travel locally for work purposes as required.
