Key Duties and Responsibilties:
Provide direct support in core areas:

- Personal Assistance: In support of the client’s care plan, assist with toileting, hygiene, showering/bathing, dressing, grooming, lifting, use of mobility equipment and other tasks such as assisting with aids or eating meals.
- Domestic Assistance: In support of the client’s care plan, undertake household duties namely vacuuming, mopping, dusting, laundry, cleaning of bathroom and toilet, food preparation and shopping assistance.
- Respite Care: Provide relief when the main care giver needs a time out for rest, recreation, appointments or administrative tasks, or needs assistance with caring for client. Respite is to be provided in a manner that is meaningful, and may include in
- home care, activities and outings.
- Social Support: Provide assistance to help service users maintain a social life within their home or accessing community based social services in meeting their need for social interaction and engagement.
- Transport: Provide transport to help service users to access medical and specialist appointments, social outings, shops and the community.
- Medication Monitoring: If needed Support Worker may monitor or supervise client taking medication in accordance with the client’s Care Plan but not to assist them with the intake of medication.

Responsibilities include:

- Contributing to developing, implementing and following programmes to assist with daily living of individuals.
- Establish appropriate working relationships by maintaining professional boundaries with clients and other service providers.
- Help promote client’s level of independence at their level of capacity and set out in their Individual Plan.
- Respect the privacy and maintain confidentiality of clients in accordance with Privacy legislation and Accessible Care and Lifestyle Services Policies and Procedures in period of employment and period when no longer in employment.
- Ensure positive image is maintained with all undertakings with clients, carers, representatives and community during the course of work hours.
- Report on observations and changes in clients’ health, behaviour, needs and circumstances to Director or Scheduling Team immediately by telephone or in person.

- Be able to drive a motor vehicle, use a computer, use mobile devices, undertake manual handling, lifting and bending.
- Effective verbal and written communication skills.
- Ability to work in a team setting and independently.
- Ability to work with others from culturally diverse backgrounds, being responsive to those differences.

Other requirements
- Pass a National Criminal Check and Working with Children Check and/or NDIS Worker Screening check
- Valid Australian Driver’s Licence.
- Roadworthy motor vehicle with minimum third-party cover (recommended full comprehensive insurance).
- Ability to be flexible with work hours to meet reasonable demands of the position.
- Inform Accessible Care and Lifestyle Services of any conditions that may impact or limit ability to carry out responsibilities required of the role.

Qualification
- Certificate III in Individual Support (Ageing, Home and Community)/ Certificate III in Individual Support (Disability) or equivalent.
- Current First Aid Certificate including CPR

**Job Types**: Full-time, Part-time

**Salary**: $25.07 - $41.90 per hour

Expected hours: 20 - 30 per week
