**Overview**

In this position, you will manage Home Care Package services under our Consumer Directed Care model, empowering clients to have greater control over their care choices. You will coordinate Home Care Package (Aged) service agreements, ensure targets are met, and provide culturally appropriate, empathetic support to clients and their families. Your responsibilities will include monitoring budgets, producing reports, and promoting Country Health Connect as a leading provider.

**Essential criteria**
- Proven experience in supervising teams, including program planning, development, implementation, evaluation, and budget management.
- Track record of meeting service obligations under formal contracts and agreements while working with government, non-government, and community groups.
- Experience in a health-related field, including developing monthly reports and managing client services.
- Comprehensive understanding of client and carer needs related to age-related disorders, relevant legislation, industrial agreements, standards, and ethics.
- Knowledge of Primary Health Care principles, Evidence-Based Practice, and Work Health and Safety regulations.

- Proven ability to work with clients across a range of disability levels and manage tasks independently while meeting deadlines.
- Strong communication skills for effective consultation, negotiation, and advocacy at various organizational levels.
- Experience in supervision, program planning, budgeting, and reporting within a multidisciplinary health-related team.
