**About the role and where you will be working**:
The Case Coordinator supports and facilitates case coordination with GPs, to increase efficiency in the delivery of quality primary, end-of-life, and palliative care (EOL&PC), and holistically improve resident outcomes and quality of life.
**We’ll also be asking you to:

- Ensure that yourself and your team abide by the Aged Care Code of Conduct and NDIS Standards acting in a way that is respectful, kind, and consistent with the behaviours set out in these documents
- Responsible for the planning, management and delivery of strategic outcomes for SCC (WA)’s Strategic Plan and works collaboratively with employees to achieve those outcomes.
- Actively contributes to a culture of continual improvement, undertaking quality improvement activities aligned with the outcomes of this project and reflective of The Aged Care Quality Standards.
- Implement the Palliative Care Outcomes Program (PACOP) with a systematic approach to routine assessments that support the identification of deterioration and palliative care needs.
- Support the development of end-of-life care capacity and capability with RAC staff.
**To be considered for this role, you’ll need**:
- Demonstrated experience delivering education training programs to clinical and non-clinical staff
- Ability to support advanced care planning for residents through discussion with residents, families and GPs.
- Ability to coordinate case conferencing with GPs, resident, families and the home.
- Ability to drive change and business improvements in collaboration with key stakeholders
