Perth Disability Services specialises in Disability Care. We are a Registered NDIS Provider delivering Disability Supports to NDIS Participants. This can range from high care, including private in-home care through to social support, daily living assistance, community access, SIL, MTA and Respite services.

**About the Role**

We are looking for Full-Time Passionate Team Members to expand our new division of Support Coordination. As a Specialist Support Coordinator, your role will be all about helping Complex NDIS participants achieve the goals set out in their NDIS plans. You’ll work closely with participants to empower them to direct their Lives. This involves assisting them in building a strong network of formal and informal supports and guiding them through complex service delivery systems. You will ensure that individual’s plan is delivered within their plan budget, supporting them to use their budget flexibly to work towards their NDIS Plan goals.
- **In order to succeed in this role, you will require the following**:_
- Previous experience as an NDIS Specialist Support Coordinator
- Diploma of Community Services or other relevant qualification
- Must be Australian PR/Citizen
- Demonstrated understanding of the complex needs and issues of people living with disabilities and their carers
- Demonstrated experience in needs assessment and management of Complex NDIS participants
- Understanding of NDIS funding instruments with experience in managing client budgets
- Excellent written and verbal communication skills
- Computer literacy
- Valid Clearance Checks
- Valid Driver License

Pay: $115,000.00 - $120,000.00 per year

Schedule:

- 8 hour shift
- Monday to Friday
- Morning shift
