**This is an identified position open only to Aboriginal and Torres Strait Islander people.**

Our Carer Gateway is an initiative to provide carers (anyone who cares for a friend or family member who is frail, ill, has disability or is an older Australian) with information and support to help them continue their fundamental role happily and healthily. Our service offers short term emergency help including Respite, Counselling and Coaching.

**About the role**

As our Identified Support Coordinator you will support all Carers from diverse cultural backgrounds who are registered in the Carer Gateway program. You will actively support our team and Aboriginal and Torres Strait Islander Carers to ensure they receive short term emergency services they desperately need.

**Duties**
- Assess and investigate the needs of the carer to point them in the right direction.
- Work collaboratively in a fast paced environment with great negotiation skills
- Develop initial service plans in collaboration with the carer, medical and allied health professionals and referring agencies as appropriate, ensuring it meets the carers needs, goals, preferences and eligibility.
- Use your analytical skills and knowledge to think outside the box to find solutions while navigating your way seamlessly through our funding systems.
- Connect and build rapport easily with clients through this predominantly phone based role to make a positive impact in our carers lives that will last a lifetime

**Skills and Experience**
- Be recognised by the Community as a culturally safe person
- Experience in connecting Aboriginal clients with appropriate services
- A strong commitment to culturally appropriate service delivery
- Experience as a Case Manager, Support Coordinator or Social Worker
- A passion for helping our clients live their best lives while they support a loved one to have a better quality of life.
- Strong emotional intelligence and the ability to create an experience where a caller quickly feels heard and comfortable, often at a time when the caller may feel emotional, confused or frustrated navigating systems they may not understand in time of emotional stress
