**About this opportunity**

We’re looking for a people-focused Care Manager to lead and coordinate care for both new and existing clients. You’ll play a key role in delivering a high-quality client experience, working closely with our office team to ensure care is responsive, compassionate, and aligned with our operational goals.

**Key Responsibilities**
- Build trust with clients and families by assessing care needs, developing personalized care plans, and coordinating services.
- Respond promptly to service enquiries and actively follow up on client leads to support growth and engagement.
- Lead client onboarding, including caregiver matching, introductions, and early-stage support to ensure smooth transitions.
- Manage Home Care Packages in line with aged care legislation, quality standards, and funding requirements.
- Collaborate with internal teams, service providers, and health professionals to deliver high-quality, holistic care.
- Conduct regular client reviews to ensure satisfaction, adjust care plans, and maintain accurate documentation in the RMS.

**About you**
- Tertiary qualifications in nursing, allied health, social work or a related health discipline.
- Experience coordinating care services for seniors, ideally within Home Care Packages.
- Strong administrative and business management skills with excellent time management.
- Confident communicator with strong relationship-building, influencing, and customer service abilities.
- Ability to work both independently and collaboratively in a fast-paced environment.
- Holds a valid driver’s license
