**About the Role**

Join Grampians Health Ballarat’s Carer Support Service (CSS) and make a difference for carers across the Central Highlands, Central Grampians, and Wimmera regions. In this 12-month fixed-term Case Manager role, you’ll work in a supportive Mt Helen-based team, helping carers set and achieve their goals through a person-centred approach.

Assess carers’ needs and develop tailored action plan

- y Engage with carers from diverse background
- s Work collaboratively with internal teams and community service

**Key Responsibilities**

Undertake carer intake, assessment, and care plannin
- g Coordinate and review support service
- s Deliver carer education and advocac
- y Build strong stakeholder and community network
- s Promote and develop carer service
- s Maintain accurate and timely client record
- s

**Skills & Experience**

Tertiary qualification in Allied Health, Nursing, or Human Service
- s Knowledge of carers’ needs, rights, and support service
- s Strong assessment, planning, and coordination skill
- s Proficient in Microsoft Office and database
- s Current unrestricted driver’s licenc
