**Role description**

Dhelkaya Health is seeking an experienced and dynamic leader to join our Senior Leadership team as the **Ambulatory Care Manager to support leave backfill for a fixed term period**. This key leadership role sits within the Continuing Care Directorate and is responsible for the strategic and operational management of a range of multidisciplinary services that support patients transitioning from hospital to outpatient and home-based care.

As Ambulatory Care Manager, you will lead the delivery of our outpatient rehabilitation programs, specialist clinics, and community-based exercise and wellbeing services. These programs are designed to restore function, promote independence, and support self-management of sub-acute and chronic conditions.

This role plays a vital part in ensuring the provision of safe, evidence-based, person-centred care that is accessible onsite, in the community, and through home outreach. You'll work collaboratively with internal teams and external stakeholders to coordinate care, optimise resources, and ensure a seamless patient journey.

We are looking for a leader with a strong background in health service delivery and a passion for value-based care. You will be committed to achieving excellent health outcomes, enhancing the consumer experience, and supporting a positive and engaged workforce.

**Key Responsibilities**:

- Lead and manage multidisciplinary ambulatory care services
- Ensure high-quality, safe, and efficient delivery of rehabilitation and restorative programs
- Promote evidence-informed practice and patient-centred care
- Oversee services across multiple settings: hospital outpatient, community, and in-home
- Collaborate across programs and with external partners to ensure integrated care delivery
- Drive continuous improvement in service delivery, outcomes, and staff experience

**About you**
- Bachelor of Nursing with current AHPRA registration or post graduate qualifications in a relevant area of leadership and management
- Minimum of 10 years' experience working professionally in a relevant clinical area
- Current valid drivers' licence
