**This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks’ notice. Are you a caring health professional ready to make a real difference for older adults with mental health needs? Central Coast Local Health District is looking for a Health Clinician Level 3 to join our Older Persons Mental Health (OPMH) Team at Wyong. This is an exciting opportunity for a newly qualified Level 3 clinician or an experienced Level 2 clinician specialising in Older Persons/Mental Health or Aged Care.

**About the Role**

As an OPMH Care Coordinator, you will work with people over 65 living with moderate to severe mental illness, including those in residential aged care. You’ll provide care coordination, case management, recovery-focused interventions, and support for families and carers. Using your advanced clinical reasoning and expertise, you’ll conduct mental health assessments, deliver evidence-based treatments, and make referrals in line with best practice standards.

You’ll work both independently and collaboratively within a multidisciplinary team, helping older adults and their families navigate complex mental health needs and improve their quality of life.

**About You**
- Hold a relevant tertiary qualification (Psychology, Clinical Psychology, Occupational Therapy, Nursing, Social Work, Counselling, or Community Welfare) with the required registration or membership and at least three years post-graduate experience in mental health, or a Master’s in Clinical Psychology with endorsement.
- Have experience in care coordination for older persons with complex needs.
- Understand the mental health needs of older adults and their families and provide sensitive, person-centred care.
- Can work independently and as part of a multidisciplinary team.
