Care Service Manager

**Home Instead** **is a leading provider of high-quality, relationship-based in-home care for ageing adults and people with degenerative conditions. Home Instead Geelong was founded by Giovanni and Giselle Siano in 2018 and now employs over 350 people in the Geelong region, with offices in Geelong West, Clifton Springs and Torquay.

We help with a range of personal and lifestyle needs while providing welcome companionship for ageing adults. Services include assistance with personal care, light household duties, meal preparation, medication reminders, and transport to appointments, shopping and social outings and care management. We also provide specialist care to people with disability and degenerative conditions, including Dementia, Parkinson's disease, MND, MS and Palliative care at home.

**About the role**

As a **Care Manager** for Home Instead you would be closely supported by an experienced team. You will be trained and mentored in how we conduct client care consultations and manage their ongoing care needs to ensure we provide quality care services to meet the needs of all our clients.

In this diverse and rewarding role you will be responsible for successfully directing and coordinating all activities associated with the care of existing clients in their own homes. This role provides leadership and management of the overall client experience and coordinates effectively with the office team to ensure we support the needs and best interests of our clients, while supporting the achievement of Home Instead operational goals.

**Skills and experience**
- Some knowledge in the aged care industry and understanding of Care Management.
- Relevant experience in a client-facing customer service or sales role.
- Excellent communication skills, including highly effective interpersonal skills and the ability to build rapport with people from all walks of life.
- Highly organised and ability to accurately document all interactions held with client, family or other stakeholders about the client in our system.
- Ability to work as an effective member of the team to ensure all needs of our clients and their families are fully supported at all time.
