**ABOUT THE ROLE**

The Home and Community Care Worker provides home care assistance and support to people with a disability, short term impairment or chronic health condition to assist them in maintaining their independence and their connection to their community. The Home and Community Care Worker provides person-centred, strengths-based services such as help with personal care, home care, support in getting out into the community, and a break for carers. Duties may include transportation, house cleaning, personal care and hygiene assistance, assisting with meals, assisting people to access and be involved in their community, and other health and wellness related activities.

The job will be on a casual basis with varying hours between 9.00 am and 5.30 pm Monday to Friday.

Successful applicants must have:

- A minimum certificate III in individual support, community services, or disability or otherwise similar qualification is required.
- Microsoft Office and typing skills
- Strong interpersonal skills including effective communication and the ability to adhere to Gateway Health’s values, policies and procedures.
- A Current Victorian Driver’s license and an ability to travel

- Strong communication and organisational skills - able to manage your time and support clients to do the same in their day to day activities.
